I hereby grant permission for the South Lake Soldiers Youth Sports Inc., program(s) sanctioned event(s), be they official or unofficial to edit, crop, or retouch such photographs, and waive any right to inspect the final photographs. I hereby consent to and permit photographs of me and/or those of my minor child to be used by South Lake Soldiers worldwide for any purpose, including educational and advertisement purposes, and in any medium, including print and electronic. I understand that South Lake Soldiers may use such photographs with or without associating names thereto. I further waive any claim for compensation of any kind for the South Lake Soldiers use or publication of photographs of me and/or those of my minor child. I hereby fully and forever discharge and release South Lake Soldiers from any claim for damages of any kind (including, but not limited to, invasion of privacy; defamation; false light or misappropriation of name, likeness or image) arising out of the use or publication of photographs of me and/or those of my minor child by South Lake Soldiers, and covenant and agree not to sue or otherwise initiate legal proceedings against the South Lake Soldiers for such use or publication on my own behalf or on behalf of my minor child. All grants of permission and consent, and all covenants, agreements and understandings contained herein are irrevocable.
By signing this form, I confirm I have read the contents of this Video and Waiver form whereas I fully understand and agree. I hereby and attest to I have all the rights, powers, and privileges, I am the parent or legal guardian of the minor child necessary to execute this document. I understand its terms and provisions, and that I have signed it knowingly and voluntarily on behalf of myself and/or my minor child.

Parent Code of Conduct Consent

As a prospective South Lake Soldier (SLS) parent/guardian or spectator, you play a crucial role in creating a positive and supportive environment for all participants. The following Code of Conduct outlines the expectations for parents and spectators to ensure a respectful and enjoyable experience for everyone involved.
Behavioral Standards
Respect: Show respect towards all players, coaches, officials, and other spectators. Your behavior sets an example for the youth.
Sportsmanship: Avoid any form of unsportsmanlike conduct, such as booing, taunting, or using offensive language.
Support: Encourage and support all players, regardless of the outcome of the game.
Positive Reinforcement: Praise your child and other players for their hard work and dedication. Avoid criticizing or ridiculing them for mistakes or losses.
Respect for Officials: Respect the decisions of coaches, referees, and officials. Refrain from questioning or confronting them on game day.
Commitment to the Team
Attendance: Ensure your child arrives on time and is prepared for all team activities. All games/practices are considered mandatory. Communicate absences with your head coach.
Communication: Communicate openly and respectfully with coaches and league officials. Address any concerns or issues to the Field Commissioner.
Involvement: Volunteer to help with team activities, such as organizing events, providing snacks, or assisting with transportation.
Social Media: Refrain from making negative remarks, or remarks that could be construed as condescending, on social media regarding the organization, coaching, directing, officiating, or board officials.
Personal Responsibilities
Role Model: Be a positive role model for your child and other players. Demonstrate the values of respect, discipline, and teamwork.
Conduct: Always maintain a high standard of conduct. Avoid any behavior that could negatively impact the players, coaching staff, spectators, or the league.
Drug & Alcohol Free: Refrain from the use of, or being under the influence thereof, drugs and/or alcohol at all SLS functions.
Field Conduct
Viewing Area: Remain in the spectator area during games and practices.
Sideline Coaching: Do not coach players during the game.
Interference: Do not interfere with a coach’s decisions during games or practices.
Player Focus: Allow the coaches to have players undivided attention during games or practices. This includes half-time and post-game de-briefing.
EJECTIONS: Any player, coach (head or assistant), or spectator who is ejected from a game by the officials, or causes a game to be forfeited due to unsportsmanlike conduct will automatically be suspended for the next game on the first offence. A second such offense will cause such player, coach, or spectator to be automatically suspended for the remainder of the season (including post-season). Any flagrant offense is cause for automatic removal from all SLS functions. No refunds will be given.
EXPULSION: Physical abuse of any parent, Game Official or SLS Commissioner by a player, coach or parent will result in expulsion of the coach/parent and their children from all current and future SLS participation. No refund will be given.
By adhering to this Code of Conduct, you will contribute to a positive and respectful atmosphere that benefits all participants in our youth sports league. Your commitment to these standards is essential for the success and integrity of our league.
I understand that if I do not uphold these above statements, that my child may be disqualified to participate within the organization – as deemed necessary by the coach, Board of Directors, or President. I further understand that no refunds issued for the actions of myself, or other family members, that contradict the working or spirit of the above Parent Code of Conduct.

As a prospective South Lake Soldier, you are expected to meet and uphold the highest standards of conduct, discipline, and responsibility. The following requirements outline the expectations for all enlistees to ensure a cohesive, respectful, and effective team:
Behavioral Standards
Respect: Show respect towards coaches, teammates, opponents, officials, team moms, and spectators always.
Discipline: Follow all team rules, regulations, and instructions.
Integrity: Be honest and have integrity.
Teamwork: Support and encourage your teammates.
Academic and Intellectual Standards
Educational Achievement: Student 1st, Athlete 2nd.
Personal Responsibilities
Daily Routine: Players are expected to maintain proper personal hygiene including regular showers, brushing teeth, and wearing deodorant to promote a pleasant environment for all.
Household Duties: Fulfill your responsibilities at home and assist with household tasks.
Time Management: Manage your time efficiently between sports, school, and home activities.
Commitment to the Team
Attendance and Punctuality: Attend all practices, games, and team activities.
Physical Fitness: Maintain a high level of physical fitness. Participate actively in training and meet all fitness standards. Hard work on and off the field = play time.
Communication: Speak respectfully with coaches, team moms, and teammates.
I understand that if I do not uphold these above statements, that my child may be disqualified to participate within the organization – as deemed necessary by the coach, Board of Directors, or President. I further understand that no refunds issued for the actions of myself, or other family members, that contradict the working or spirit of the above Player Code of Conduct.
